Bariatric Pre-Operative Evaluation

Includes Assessment, Report and Clearance Determination

We are looking forward to working with you for the psychological clearance portion of your bariatric pre-operative clearance process. Our Evaluation Coordinator will call you to schedule two appointments for the clearance evaluation after receiving a referral from your bariatric provider. During the first appointment, you will be asked to fill out extensive questionnaires regarding your mental and physical health. During the second appointment you will meet with Dr. Forsyth to complete an in-depth interview regarding your mental health and preparedness for surgery. This appointment typically takes 1.5-2 hours to complete. At the end of this appointment, Dr. Forsyth will discuss your clearance for surgery with you. After your appointments. Dr. Forsyth will also complete and send you a formal report. 

*Please note that our office is aware that the psychological clearance evaluation is typically one of the last steps in the process before being able to schedule your surgery date, and we work diligently to provide thorough and comprehensive services within a timely manner. Our Bariatric Evaluation timelines are expedited from the timelines for other evaluations here at Freedom First but can vary.
